Clinical Massage

Peak Function Health is a clinical massage practice, specializing in orthopedic massage.  Our primary focus is on acute injury care, pain management and injury prevention.

Clinical massage is considered a general term used to indicate massage which used in a healthcare setting. It can include a variety of modalities, settings, client populations, health concerns, and approaches. It utilizes an outcome-based approach where goals are established through consultation with the client or possibly with one or more healthcare professionals. Clinical massage usually involves assessment and treatments formulated for specific medical conditions. These conditions can range from muscle injury/ movement, mental health, oncology or other health challenges.  

Orthopedic massage is a type of clinical massage whose primary focus is on movement/ structural related conditions.  It is used to help address muscle pain, acute injury, and movement dysfunction.  Orthopedic massage can also be used to help reduce injury risk by identifying and targeting existing movement dysfunctions and muscle imbalances that haven’t yet led to dysfunction such as pain or movement limitation.

We specialize in injury and post surgical care.  We also enjoy helping active people maintain their active lifestyles, at their highest possible level, and for as long as possible. All sessions are tailored to each person’s unique needs.      

All levels of tissue, from superficial to deep, contribute to the function of the movement system.  Different tissue types need different treatment approaches such as muscle, fasciae, ligaments, and nerve tissue. We use both direct and gentle approaches, static and active movement techniques, and target different tissue depths. The goal here is to help you regain and maintain the best soft tissue and movement health possible.

New Client Special (Cash/ Credit Card/ ATOS):  $75/ 60 minutes

Session duration includes both bodywork and intake.  Bodywork time varies depending on the amount of time needed to do the intake.

Peak Function Health gladly accepts Cash/ Credit Card, HSA, and FSA.

We accept Auto Accident (PIP) primary insurance.

We are in-network providers with Premera, Aetna, Kaiser Permanente. To book, you will need to contact us directly. Verification of insurance and prescription (with diagnosis codes) is required before beginning care.

We ask for 24-hour notice for cancellations.  By booking an appointment, we are reserving a spot for you. Late cancels can be challenging to fill. Fees apply for late cancel ($50) and No-show ($75). Thank you for understanding.

First Visit (Cash/Credit Card/ ATOS)

Dan first priority is to get acquainted with you, and discover how he can best help you to realize your health goals.  Extra time is worked into the first visit for that purpose.  Please plan for up to 75 minutes for this appointment. 

Please make sure to choose "First Visit (Cash/ CC)" when you book you first appointment.

Auto Accident

The first appointment includes a comprehensive intake and assessment. With subsequent visits, the intake-assessment time is usually shorter.  

For your first appointment, please plan for up to 90 minutes. 

We accept 1st party Personal Injury Protection (PIP/ Auto Insurance) policies only.

Insurance

Peak Function Health accepts 1st party PIP Insurance (Automobile Accident). We do not accept 3rd party policies.
We also accept Kaiser Permanente Health Insurance. We are an in-network provider for Premera, Aetna, and Kaiser Permanente.  

Insurance Policies

For motor vehicle accident (PIP) claims, we need your:

  • Company Name
  • Policy Number
  • Maximum Amount of Coverage & Current Status (PIP)
  • Adjuster Name 
  • Adjuster’s Phone Number and Email Address
  • Mailing Address to Send Claims
  • Date of Accident/ Event 
  • Referral from a Qualifying Medical Provider such as a Medical Doctor, Chiropractor, or Physical Therapist. 
  • Referral needs to include Diagnosis Codes

We cannot begin massage therapy without these items received and confirmed.

The client is responsible for any denied claims. A signed agreement acknowledging and confirming this responsibility is required before your first massage appointment.

Major Medical Insurance

We are in-network with Premera, Aetna, and Kaiser Permanente.

All insurance claims require medical necessity to use. They can not be used for maintenance or preventive services. Referral for massage therapy by a qualifying medical provider such as a Primary Care Physician, Chiropractor, or Physical Therapist is required even if your policy does not require a preauthorization.

All claims require diagnosis codes. Since, Massage Therapists are not allowed to diagnose any condition, the diagnosis codes need to be obtained from a qualifying medical provider.

We need the following information for both In-Network and Superbill claims.

  • Referral with Diagnosis Codes
  • Insurance Company Name
  • Policy Number
  • Group Number (if applicable)

If you massage therapy benefits with a carrier that we are not in-network with,  direct reimbursement from your insurance company may be possible.

For all out-of-network services rendered, full service payment is required at time of the appointment.  Afterwards, we can provide you with a Superbill for you to submit to your insurance company.  A Superbill is a special itemized receipt of services performed that can used to submit a claim for possible reimbursement.

You should contact your insurance company about the following information to help minimize unwanted surprises.   

Do you have out-of-network benefits for Massage Therapy? How much coverage do you (Out-of-Network)?  What is your deductible and what is its status? Can massage therapy be rendered by a massage therapist?  Some policies will only cover massage therapy services rendered by Physical Therapist or Chiropractor.  

Is it a combined or solo benefit?  Some policies combine services from multiple specialties under one benefit.  Physical Therapy and Chiropractic are commonly grouped with Massage Therapy.  If you are receiving care from multiple specialties that are covered by the same benefit, the max visit/ coverage allowed will be exhausted faster.

We cannot guarantee that your insurance company will reimburse you for services rendered.  Claims are denied for various reasons.  Follow-up with Insurance pertaining to a denial is the responsibility of the client.
